Registration for a class in our training center is complete once you’ve submitted the online registration and your payment has been received.

Although using our online payment system is considerably faster and more reliable, both check and cash are also acceptable forms of payment. Check and cash payments may be paid at the time of your class or submitte to our office anytime before the class meets.

Payments well in advance of the class is highly advised. If you are told to come to class and pay then, you will have a spot.

Mail or hand-deliver your payment to our training center:

• 1351 Collyer St. Longmont, CO 80501 (We are located in the Longmont Church of Christ building)

If paying by check, make it out to:

• Hopelight Medical Clinic

On the front of the check, write:

• one phone number
• your driver’s license number

Besides your check or cash, place a note inside the envelope with the:

• name and date of the requested class
• name, phone number, and e-mail address of the person who will be attending class
